Please read through all policies before booking an event. Please note that when you make a booking you are agreeing to accept these terms and conditions.

Participant Declaration

Attention: All participants entering the facility must comply with this declaration. 

 By booking I have accepted this document. I agree to follow all directives, and engage with all requirements in Axewood’s COVID-19 Safety Plan. Additionally, I hereby acknowledge and agree to respect the following information outlined in this document: 

1) Sickness

a) I will stay home if I am unwell, or if someone in my household is unwell, or is displaying the following symptoms:

i) Fever and chills

ii) Cough 

iii) Shortness of breath

iv) Sore throat and painful swallowing

v) Stuffy or runny nose

vi) Loss of sense of smell 

vii) Headache 

viii) Fatigue, etc. 

b) I confirm that I have not knowingly been in contact with a person that has a confirmed or suspected case of COVID-19. 

c) I agree to complete a routine daily screening process prior to entering the premesis. 

d) I confirm that I have not travelled outside of Canada in the last 14 days. Additionally, I confirm that I have not been knowingly exposed to someone who has travelled outside of Canada in the last 14 days.

e) I acknowledge there are inherent risks associated with participating in the booked activities. By attending the booked activities, I understand and assume all risks associated with potential exposure of COVID-19. 

2) Personal hygiene:

a) I agree to follow all personal hygiene requirements set out at the studio, including but not limited to: frequent hand-washing and sanitizing, coughing and sneezing into my sleeve, etc.

3) Physical distancing

a) I agree to practice safe social interactions, by maintaining a minimum distance of two meters between myself and others. 

4) Environmental hygiene

a) I agree to adhere to all studio cleaning requirements. 

5) Physical modifications 

a) I understand that equipment may be moved in order to facilitate safe social interactions and physical distancing. Additionally, I understand and agree that if I do not adhere to the requirements set out by Axewood, I may be asked to leave the premises  in order to protect the health and safety of all involved. This Participant Declaration of Compliance will remain in effect until Axewood determines it is no longer required, based on viaSport, PHO, and WorkSafeBC requirements. I also confirm that I have signed (Parent or Guardian if participant is under age 19) the Release of liability, waiver of claims, assumption of risks and indemnity agreement. 


Fees, Refunds, Cancellation Policies and Covid-19 Considerations

Deposits

A 50% deposit is required on booking to secure your allotted booking time slot. All deposits are non-refundable and non-transferable. Once payment has been made for the session we cannot refund you if your circumstances change. There are limited spaces during covid and we have to staff events in advance. If you have not paid in full before the session commences you will not be allowed to participate in an event until you have completed the payment. 

 

If you become injured before the session and are unable to participate in an event for which you have paid the deposit, you must provide a doctor’s note in order to be considered for a credit on that single person’s account. This credit if determined eligible will be applied to your online account for future use. If eligible we provide a credit on account only, we do not apply a refund.

 

Covid Considerations

 

A brief closure

 In the event that the facility must close for 2 weeks for a confirmed case of covid, whether participant or staff, we will report to Vancouver Coastal Health immediately and follow directions on how to proceed. We will be transparent and keep you updated on any situation that occurs. In this scenario based on the instructions given by VCH we will resort to one of the following;

- if time permits you will be able to reschedule. 

- credit applied to your account for the total of the deposit paid on booking. 

Please note we will not be providing a refund for experiences in the scenario of a two week shut down.

 A government mandated closure 

 In the event of a government enforced lockdown we will be offering a credit on account only. We will not be offering a refund. Please make sure you are 100% aware of this before committing to booking and paying the deposit. By accepting this agreement you are acknowledging that you are registering for an experience knowing this information if the situation changes. 

A government mandated partial closure or change of operation

 In the event of a government enforced partial closure or change of operation, we will run experiences in an altered way depending on the government guidelines. If the government allows us to remain open with changed practises, these changes will take place immediately and will replace all usual processes. If we are able to remain open, we will, even if this is at a different set up to the usual offering. No credits or refunds will be given due to this change. 

 Illness or Sickness Covid-19 Related 

 In the event that you feel unwell and have a covid symptoms but have not been tested for covid or are awaiting a test result from a covid test please do not come to the facility. Please contact us so we can reschedule your activity if possible.

 Isolation

 In the event that you are required to self isolate due to contracting covid-19 yourself, being in contact with someone with covid-19, travelling out of or back into Canada, a family member, friend or close contact as been in contact with someone with covid-19, or any other reason that Vancouver Coastal Health has required you to quarantine or self- isolate, a virtual viewing of the missed class will be available to you. Credits or refunds will not be given to those who have been required to isolate or quarantine.

 Choosing Not to Attend an Event

 In the event that a customer chooses not to come to an event as they have an immunocompromised family member, friend or housemate or if your feelings about coming to an event during this pandemic change while the facility is still open and able to run events, credits or refunds will not be given.

 Testing for Covid-19

 In the event that you yourself are being tested for Covid-19, we require you to alert our team on our email crew@axewoodcrew.com. Before you are able to participate in an event, you must let us know your timeline even if no one else on the premises are likely to have contracted. Before attending please let us know you are coming, what your timeline is  including when you may have contracted COVID, how long you have been isolating for and when you were approved by VCH to stop isolating. We require this information in writing via email prior to attending an event.


Media Waiver

I acknowledge and consent to allow Axewood to use: photos and/or videos of my child/myself in publications (such as flyers/brochures), advertisements (such as newspapers or online ads), our company website, or on social media sites (such as the facebook or instagram account). I understand that in some cases, my child’s and/or my name may be used. I further understand no financial compensation will be given for use of these photos/videos.
